Please ensure that you as the legal parent/guardian of the student complete ALL the information below correctly.

This enrolment form constitutes part of the terms and conditions for enrolling in and attending classes at Stars Performing Arts Academy (“Stars”). We provide you with this document as part of our mission to give all students attending Stars the opportunity to have fun and learn to dance, sing, act and acro.

A registration fee of $20 per child is required to confirm enrolment. This fee includes administration and insurance expenses for your child.
